Thank you for choosing Mind Health, LLC. Counseling is a collaborative process designed to help you improve emotional well-being, strengthen relationships, develop healthier patterns, and create meaningful, lasting change.
My approach integrates evidence-based counseling with practical strategies that support resilience, healthy relationships, and intentional living. Discussions about sleep, nutrition, movement, stress, and lifestyle are educational and are not a substitute for medical care.
Practitioner: Jodi Perelmuter, Pre Licensed Professional Counselor (LPC) in Kansas.
Jodi is awaiting full licensure and currently practices under the supervision of Catherine Grabow, LSCSW (Clinical Social Worker, Overland Park, KS; NPI 1124779822). Counseling services are provided within the scope of professional counseling. Jodi does not practice medicine or prescribe medication. If she later earns a Doctor of Behavioral Health (DBH) degree, it is a non-medical doctoral degree and does not confer physician status.
Counseling may improve insight, coping skills, relationships, and overall well-being. Therapy may also involve uncomfortable emotions or difficult conversations. Outcomes cannot be guaranteed.
Your information is confidential except when disclosure is required or permitted by law, including concerns about safety, suspected abuse or neglect, court orders, certain healthcare operations, or with your written authorization. You will also receive a separate Notice of Privacy Practices (below).
This notice describes how protected health information (PHI) about you may be used and disclosed by Mind Health, and your rights regarding that information, as required under the Health Insurance Portability and Accountability Act (HIPAA).
A full written copy of our Notice of Privacy Practices is available on request. We reserve the right to change this notice and will provide an updated copy upon request.
Under the No Surprises Act, self-pay and uninsured clients have the right to receive a "Good Faith Estimate" explaining how much your care will cost.

Payment is due at the time of service unless otherwise arranged. Self-pay clients have the right to receive a Good Faith Estimate under the No Surprises Act. Missed appointment and late cancellation fees apply according to practice policy.
Sessions are reserved specifically for you. We require at least 24 hours' notice for cancellations or rescheduling. Cancellations with less than 24 hours' notice, or missed appointments, may be charged the full session fee. Accepted forms of payment include major credit cards processed securely through our booking system.
Illness policy: I live with an immunocompromised person. Please do not come to an in-person session if you are feeling unwell or have any symptoms of illness. Instead, move your appointment to a virtual (telehealth) session, or I can cancel your appointment at no cost to you. There is no cancellation fee when you contact us due to illness.
Our relationship is limited to the therapeutic setting. I do not accept personal social media requests. If we meet in public, I will not initiate contact in order to protect your privacy.
Email, text, and voicemail are for scheduling and administrative matters only. They are not continuously monitored and should not be used for emergencies or urgent clinical concerns. For your protection, please avoid sharing sensitive health details over unencrypted channels.
Telehealth services are available for clients physically located in Kansas. At each session, you may be asked to confirm your location and an emergency contact. Technology interruptions may occur.
Telehealth uses secure, encrypted technology to protect your privacy. There are potential risks, including technology failures and limitations of remote assessment. You may withdraw consent for telehealth at any time without affecting your right to future care, and in an emergency, telehealth is not a substitute for in-person or emergency services.

Reports, letters, record reviews, court testimony, and other services outside routine counseling may be billed separately.
Counseling may end when treatment goals are met, services are no longer appropriate, attendance becomes inconsistent, or financial obligations remain unresolved. Referrals will be provided when appropriate.
